Privacy Notice

Being transparent and providing accessible information to individuals about how CPIMS (the data controller)  will use their personal data is a key element of the Data Protection Act 1998 (DPA) and the EU General Data Protection Regulation (GDPR). Personal information provided or collected via our website, or other methods of contact, will be treated confidentially and will be dealt with in accordance with this Privacy Notice. Any personal data you provide to CPIMS will be used by us to deal with any enquiry you make through our website, or other methods of contact, to provide any services which you instruct us to provide under a contractual agreement.

 

Cookies are small text files stored in your browser and are used by most websites. CPIMS use cookies to collect information about how visitors use our website. IP addresses are stored when a visitor accesses our website. Your Personal Information

Personal information we may hold about you, or that has been provided to us, in relation to services we provide, under a contractual agreement, may include:

  • Name, address,  and date of birth.
  • Your email address and telephone number.
  • Medical records, reports and treatment notes including photographs and videos that relate to treatment or progress regarding the services we provide under a contractual agreement. Such records are never shared without your consent.

 

CPIMS use this information

  • to communicate with you, or other instructing parties, regarding services provided by CPIMS under a contractual agreement.
  • to obtain your views and opinions about the services we provided to you.

 

Purpose of Sharing

CPIMS will use your information for the purpose of providing a service agreed under a contractual agreement.

With your consent the sharing of information between medical professionals and allied healthcare agencies, involved with an individual’s care, aims to share information relevant to an individual’s treatment plan. Information may be shared with CPIMS to support day-to-day business which underpins the delivery of the service;  to meet legal requirements, including requests from patients under subject access provisions of the Data Protection Act 1998 or the Freedom of Information Act 2000; to support our clients’ choice and control over treatment and services designed around the client; to comply with Court Orders or other requests for information in accordance with the law at any given time. You will be notified of any changes to the law which affect this policy.

We aim to keep our data as accurate as possible. If you wish to update or review your data you should email the Data Processing Officer. You can withdraw your consent at any time by emailing the Data Processing Officer: Denise Watling denise@cpims.com. CPIMS will never share your personal data with a third party for marketing purposes.

 

Public Forums and Social Media

Any comments you post on our blogs, audit forms or social media pages is public information and you should consider what personal information you post on such platforms.

 

Security

The internet is not totally safe and secure. On occasion information sent over the internet can be intercepted. Whilst we store our data on secure servers protected with firewalls and virus protection software we cannot guarantee the security of the data you send to us electronically. We use portals on secure servers to store data specifically for sharing. Users will be required to logon with user names and passwords and are specifically asked not to disclose those passwords to other individuals. We expect users to show the same due diligence. If you use services outside of the European Union (EU), the information you provide may be transferred to Countries outside of the EU, and the data protection laws may vary. However, we will take all necessary steps to ensure your Privacy rights continue to be protected in accordance with this privacy notice.
